Diabetes Self-Management

Similar to Better Choices, Better Health, the Diabetes Self-Management Program is a chronic disease self-management program designed for those struggling to manage their diabetes. The workshop environment allows older adults to discuss strategies for managing their illness with peers and trained instructors to help facilitate discussion and other activities. This program also benefits individuals who care for someone living with diabetes.

Discussion Topics Include:

  • Techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, and emotional problems such as depression, anger, fear and frustration

  • Appropriate exercise for maintaining and improving strength and endurance

  • Healthy eating

  • Appropriate use of medications

  • Working more effectively with healthcare providers

Class Structure:

Diabetes Self-Management Program is offered 1 time per week for 6 weeks.  Each session lasts 2.5 hours.
