The Silicon Valley Healthy Aging Partnership (SVHAP) was formed to help bring Evidence-Based Programs for health and wellness to the older adult populations in Santa Clara County.  We assist in coordination of national health programs with local community partners to create a culture of physical activity for area older adults.  These partnerships and programs of SVHAP connect the diverse communities and capabilities represented in Santa Clara County agencies to health programming that is both affordable and accessible. In addition, we coordinate resources and leverage funding to maintain high-quality programs that will reduce health disparities in our community.
